Job Summary

Connect2Health (C2H) Interns will work closely with University of Utah Health Plan’s ( UUHP ) care management team to reach out to new members, conduct screenings, introduce care management program, complete the Health Risk Assessments and provide community resources for those needing additional support. C2H Health Interns will receive a training documenting patient interactions in the EPIC Tapestry and Care Management Module systems. C2H Interns will collaborate with a team of nurses and coordinators on a regular basis.

A C2H Intern’s workspace may be in-office at a cubicle with a desk top computer or remotely from intern’s home during COVID conditions. There may be occasional home visits with UUHP representative/colleague and in-person interactions with members at local community clinic.


A. Telephonic outreach to new members enrolled with UUHP .

B. Introduce UUHP care management program and request consent to complete a scripted Health Risk Assessment ( HRA ).

C. Input member responses into Epic Tapestry and/or MHK .

D. Work with UUHP care teams to educate them on Connect2Health community resources library/website.

E. Provide community resources to UUHP members that are in need or inquiring about possible services. This is done through curating a resource prescription through the Connect2Health database

F. Utilize the Connect2Health database to input member interactions, research previous resources or interactions, and follow up with members on given resources.

G. UUHP will provide training and education about UUHP history, insurance products, plan design, care management services and other departmental aspects in UUHP .

H. Reporting responsibilities to the Care Management Manager and Project Coordinator.

I. Complete discharge dates by calling hospitals and referring to a daily spreadsheet given by Utilization Management.

J. Attend team huddles and Clinical Operations meetings as appropriate and agreed upon by management and the assessment/outreach team.

K. Notification of absences or time off requests needs to be communicated to CM Manager.

L. Other duties as assigned.

Minimum Qualifications

This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. A. Self-starter with minimal need for intervention or supervision.

B. Excellent oral and written communication skills and the ability to appropriately adjust communication for the audience.

C. Strong interpersonal relationship skills, effective listener and knowledge of motivational interviewing techniques.

D. Strong interest in working with a team.

E. Ability to think “outside of the box” for problem solving issues to support a member in identifying resources that may help be safe and as healthy as possible while remaining in the community.

F. Fluent in Spanish.

G. Student at the University of Utah pursuing degree in a health related field.

H. Preferred if applicant owns a personal laptop that will be utilized for work related tasks, during work from home status.
