The Division of Cardiovascular Medicine is looking for a motivated and enthusiastic individual to work on our team. The ideal candidate is someone who has been practicing as a Nurse Practitioner for at least one year and who has experience with cardiac patients.

Participate in daily inpatient rounds (M-F) on the HF/Transplant/ VAD service.
- participate in weekend call
- coordinate care across specialists
- ensure discharge criteria are met
- ensure timely discharges take place
- work with case managers to ensure effective post-acute care planning
- ensure consistent communication with referring providers
- work closely with CVMU nurses, preventive care services and providers to identify opportunities for reduction of readmission rate
- identify opportunities for improved patient satisfaction
- work closely with team to determine whether variations in inpatient and outpatient care result in opportunities for changes aimed increasing value of care provided on the HF service
- the APC will complement a group of 3 other APCs who are part of the HF team; a rotation of (new) inpatient and (current) outpatient responsibilities will be implemented among the APC team.
Licensure to practice as a Nurse Practitioner in the State of Utah. Nationally certified as a Nurse Practitioner as required under State Law. Certification in ACLS / BLS / NRP / ATLS may be required by your department or by privileging criteria. Demonstrated clinical competence and in-depth knowledge of specialty area; and demonstrated leadership, teaching, research and communication skills are required.
Utah State prescriptive privileges preferred.
Licensure is obtained by completing a Master’s degree in Nursing or equivalency (the only equivalency to the Master’s degree is current licensure as a Nurse Practitioner) and a National Certification which requires the successful completion of a nationally accredited examination. UHPPM 1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire.
This position is patient-sensitive and must fulfill all associated requirements. One year of experience as a nurse practitioner.
