Job Summary

Utah Adult Congenital Heart Disease ( ACHD ) Program is seeking a Nurse Coordinator to join our team. This program is dedicated to the ongoing care of adults born with heart defects and other genetic disorders that affect heart function. The Utah ACHD Program is certified by the International Society of Adult Congenital Heart Disease and is a referral center for Utah and surrounding states. The Nurse Coordinator position requires administrative work as well as one-on-one time with patients and families. We are looking for an individual who desires to be part of a dedicated integrated team and demonstrates strong assessment, problem solving and communication skills.

Responsibilities

I. Clinic Coordination (1-2 clinics per week)

A. Pre-clinic

•Analyze diagnoses, history and patient symptoms and determine and order needed testing, additional records and timing of appointments

•Compile and email patient summaries to providers, rotation fellows, residents and students before clinic

•Communicate of testing needs with clinic medical assistants

B. Clinic

•Perform initial screening and identify patient status, needs and concerns

•See patients with cardiologist to build rapport and maintain continuity of care

•Assure all patient questions, needs and concerns are addressed with a plan of care

•Assess patient understanding

•Provide patient education, specific instructions and counseling

•Facilitate the flow of clinic and assure patients are seen in a timely manner

C. Post-clinic

•Order and schedule any additional tests, new medications and return to clinic orders

•Provide post-clinic summaries to assure all needed issues are addressed and responsibilities defined

•Provide nursing phone calls and or visits for follow-up needs and communicate test results

II. Core Daily Activities

•Manage patient phone calls and patient portal questions

•Triage cardiac symptom

•Document significant patient care issues between clinics visits [complaint(s), assessment, plan and follow-up in the electronic medical record (MyChart)]

•Coordinate patient care with other sub-specialties and providers

•Manage medication : e-scribe, refills, titration based on provide’s instructions, monitoring labs indicated for medication management, and patient education

•Coordinate inpatient and outpatient interface

•Monitor for non-compliance (missed appointments, refill renewals, labs work, testing, etc.)

•Serve as a resource for ongoing care information, counseling and emotional support

•Write letters and fill out forms as requested by patient

III. Program Collaboration

•Monthly ACHD Meetings

Formulate patient data summaries

•Monthly ACHD and Cardiac Pregnancy Meeting

Help maintain and update current pregnancy list

•Maintain patient research information

•Participate in community education programs

•Contribute to Quarterly ACHD Team Meeting

Minimum Qualifications

Current licensure to practice as a Registered Nurse in the State of Utah or equivalency required. *If licensed in a compact state a Utah State license must be obtained within 90 days of hire.

Basic Life Support Health Care Provider card or must obtain one within one month of hire required.

Strong organizational and leadership skills as well as demonstrated human relation and effective communication skills are also required.

Some positions may require additional certifications such as Advanced Cardiac Life Support ( ACLS ), Pediatric Advanced Life Support ( PALS ), or Neonatal Resuscitation Program ( NRP ) within one year of hire.

This position is patient-sensitive and must fulfill all associated requirements. Preferences

•Bachelor’s degree in Nursing or equivalent cardiovascular experience

•One-year cardiovascular experience

•Strong written and interpersonal communication skills

•Organization and technical skills (experience with of i Centra and/or EPIC electronic medical record platforms, helpful, but not required)

•An ability to establish rapport with patients, their support systems, and other health professionals

•A desire to work in a collaborative team model
