Job Summary
The Patient Care Coordinator ( PCC ) plays a key role in the practice by working with practice teams, patients, and in building systems, (re)designing processes, leveraging health information technology and collaborating to improve the quality, equity, and outcomes of care. In addition, the PCC will provide care coordination, as well as facilitate the provision of care activities as specified by members of the practice team.
Responsibilities
•Communicate and collaborate between patients and the practice’s care teams. Facilitate excellent patient care. Maintain patient care database by entering new information as it becomes available, verifying findings. Schedule surgeries and pre-operative testing, coordinating with various internal and external care teams. Provide follow-up and support as needed with patients at the request of the physician and care teams. Assist patients in navigating the healthcare system. Responsible for tracking clinic referrals and auditing referrals periodically to verify appropriate follow-up.
•Quality Improvement: Participate in the use of quality improvement techniques and tools to:
oIdentify relevant improvement opportunities
oDesign and implement improvements
oMeasure their impact
oHold the gains
oSeek opportunities to continuously improve
•Collaborate on various tasks including, but not limited to; patient referrals, patient resources, process improvement, facilitating care as defined by the physician and APC teams.
Minimum Qualifications
Bachelor‘s degree or equivalency in public health, health education, health promotion, management, behavior science, social science, social work or nursing with 2 years relevant work experience or Medical Assistant with State of Utah, National Certification, or equivalency and 5 years of clinical experience.
Demonstrated ability to lead as well as function effectively as a member of a team and successfully promote change. Strong analytical, organization and leadership skills. PC/Windows literate with working knowledge of the following Microsoft applications: Outlook, Word and Excel. Demonstrated human relation and effective communication skills.
This is a patient-sensitve position.
Preferred Experience
Working knowledge of quality improvement and project management methodologies, tools and techniques and principles of change management. Demonstrated ability to effectively facilitate and mentor project teams preferably using QI methods (i.e., PDSA ). Familiarity with electronic medical record ( EMR ) applications.
