Overview

PATIENT NAVIGATOR for the University of Utah Transgender Health Program

The University of Utah Transgender Health Program is a multi-disciplinary program that serves to provide and facilitate the heath care needs of gender nonconforming individuals. The Program includes specialists in family medicine, pediatrics, adolescent medicine, endocrinology, plastic surgery, urology, speech-language pathology, physical therapy, gynecology, fertility, and mental health.

The Patient Navigator will have the primary responsibility of assisting patients in accessing care in the Transgender Health Program. They will be responsible for receiving and managing patient requests for information, and for coordinating care between departments and providers. They will perform patient intake interviews and work with staff in scheduling appointments, scheduling surgery, and coordinating pre and post op care. The will serve as a liaison for patients while hospitalized to ensure excellent patient experience. Additionally, the will assist the Program Coordinator with organizing meetings, outreach events, and education seminars.

Typical hours will be 8am-5pm, although early, late, or weekend hours will be occasionally necessary.

Responsibilities

  • Receives, responds to, and directs patient communications, including phone calls, emails, and other interactions.

  • Assesses patient questions and ensures the appropriate coordination of the patient experience. Directs the scheduling of patient appointments with providers and ancillary services.

  • Educates patients about available resources and what to expect during their visit.

  • Coordinates referrals from physicians and self-referred patients.

  • Gathers needed medical information such as records, slides, films, and ancillary tests to assist in the coordination of patient needs.

  • Ensures insurance coverage, resolves any issues with coverage, and escalates complicated issues to appropriate financial counselor.

  • Monitors program performance and coordinates any programmatic development within the team.

  • May assist clinical staff in patient procedures in accordance with the incumbent's licensure, as required by the specific department.

  • May coordinate treatment planning conferences, logistics, presentations, and associated CME requirements.

  • May act as a liaison with other care groups such as social work, financial counseling and nutrition to facilitate the patient intake process.

  • May assist in the coordination of seminars and community outreach activities.

  • May assist in the procurement of the appropriate patient supplies and equipment.

Knowledge / Skills / Abilities

  • Demonstrated excellent communication, interpersonal, organizational, and follow-through skills.

  • Ability to be motivated with attention to detail and the ability to work efficiently and independently, demonstrating a passion for providing excellent customer service.

  • Ability to learn new applications and software, procedures and processes.

  • Ability to assimilate data from various sources.

  • Ability to display active listening and verbalize empathy while developing relationships with patients, guests and coworkers.

  • Able to prioritize and multi-task in a fast paced environment.

  • Demonstrated knowledge of HIPAA regulations to ensure that patient information is guarded and respected.

Qualifications

Qualifications Required

  • Associate's degree in a Health Care, Business, Communications, related field, or equivalency.

  • Two years of applicable experience.

Qualifications (Preferred)

Required

  • Mental Health/counselor training (LCSW, MSW, CMHC)

  • Experience in healthcare disciplines, ideally including care coordination, case management, community mental health, patient education and complex social issues, working with an adult and adolescent underserved population

  • Must have valid driver’s license.

Preferred

  • LGBTQ community experience, including an understanding of biopsychosocial issues affecting transgender populations, such as healthcare needs, substance abuse, health risks, and legal issues.

  • Experience with electronic medical record (EPIC)

  • Competency in electronic patient charting and scheduling.

  • Some departments may prefer experience in a high volume clinical setting or multidisciplinary clinics or working with high-acuity, complex patients.

  • This is a sedentary position that may exert up to 10 pounds and may lift, carry, push, pull or otherwise move objects. This position involves sitting most of the time and is not exposed to adverse environmental conditions.
