Candidate must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients being seen in the integrated sports/spine program. The individual must demonstrate knowledge and the ability to assess data regarding the patient’s status and provide care as described.

1. Evaluation and treatment in sports/spine care.
2. Will work closely with, and under the supervision of, a Physical Medicine & Rehabilitation (PM&R) physician.
3. Takes complex medical histories on patients referred for consultation by academic and community physicians.
4. Orders consultations and interacts with members of other specialty medical teams, particularly neurosurgery and orthopedic surgery.
5. Performs physical examinations on new and return spine patients.
6. Orders laboratory tests, x-rays, special imaging studies and other diagnostic procedures.
7. Performs trigger point injections, bursa injections as needed for treatment of related pain disorders.
8. Monitors dosages and adjusts medications.
9. Orders and supervises the administration of physical therapy.
10. Reaches tentative diagnosis of medical, surgical and other conditions.
11. Develops treatment plans in conjunction with attending physician to provide optimal patient medical management.
12. Participates in continuing medical education to include specialty and sub-specialty meetings, clubs, boards and conferences.
13. Willingness to participate in quality improvement activities.
14. Counsels patients and families about health wellness, illnes and maintenance.
15. May assist in research projects.

Work Environment and Level of Frequency that may be required:
Nearly Continuously: Office environment.
Often: Infectious disease, oils ( there is air or skin exposure to oils or other cutting fluids ), hazards ( includes a variety of physical conditions, such as proximity to moving mechanical parts, moving vehicles, electrical current, working on scaffolding and high places, or exposure to chemicals ).

Licensure to practice as a Nurse Practitioner in the State of Utah. Nationally certified as a Nurse Practitioner as required under State Law. Certification inACLS/BLS/NRP/ATLSmay be required by your department or by privileging criteria. Demonstrated clinical competence and in-depth knowledge of specialty area; and demonstrated leadership, teaching, research and communication skills are required.
Utah State prescriptive privileges preferred.
Licensure is obtained by completing a Master’s degree in Nursing or equivalency (the only equivalency to the Master’s degree is current licensure as a Nurse Practitioner) and a National Certification which requires the successful completion of a nationally accredited examination.UHPPM1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire.
