Job Summary

Compiles, assembles and organizes patient medical records in department files and in the department’s computer system and makes record information available to staff, providers and patients. Ensures compliance with medical recordkeeping policies and researches and resolves any problems with medical records. Responsible for the ongoing digitization/archiving of historical paper medical records.


Essential Functions

  • Completes external provider records requests for multiple divisions within Women’s Health
  • Responds to patient calls & EPIC inbox messages
  • Creates, sends, and follows up on any outgoing medical records requests
  • Upload and organize external medical records into EPIC
  • Requests images and coordinates with Radiology to convert and add into patient charts
  • Coordinates the process for Consent for Sterilization forms
  • Creating invoices for insurance medical requests
  • Organizes patient documents for entry into EPIC
  • Scans documents according to date and procedure performed and follows recommended document naming convention standards (including historical paper records)
  • Performs quality checks to confirm accuracy of import process
  • Destroys scanned paper documents according to organizational policies and procedures
  • Recover records from the online database to forward to those requesting records
  • Maintains high productivity and stays on task
  • Collaborates with co-workers to fulfill workload duties as needed within the Department
  • Additional duties as assigned

Minimum Qualifications


Two years of secretarial experience, or equivalency.

Successful completion of medical software training may be required by a certain date within some departments.


Two years of experience in a hospital, clinic or laboratory setting.

Patient registration or related field experience.
