University of Utah Health Health Unit Coordinator in SALT LAKE CITY, Utah

Overview

This position will work at UNI on Youth Services and will provide assistance to the clinical social work staff. Any previous experience in mental health care is a plus.

Responsibilities

  • Identify and obtain appointments for new referrals. Make follow up appointments with establish providers.

  • Request records from outside providers such as therapists, psychiatrist, residential programs or schools.

  • Fax clinical records to aftercare referrals and established providers

  • Prepare letters re: hospitalization (as appropriate)

  • Update social work assignment boards, TeleXT, informational postings

  • Completes reports and work queues as assigned

  • Keep updated records on insurance coverage for various referrals

  • Attend weekly social work staff meeting, trainings, etc.. As applicable

  • Maintain open communication and continuous training with the records department and social work staff about matters pertaining to ROIs and continuation of care.

  • Prepare release of information forms and update in EPIC (Outside Access)

  • Perform duties as the point of contact between day treatment and inpatient services at UNI

Knowledge / Skills / Abilities

  • Demonstrated potential ability to perform the essential functions as outlined above.

  • Demonstrated human relations and effective communication skills.

  • Ability to successfully complete power chart training.

  • Demonstrated ability to use a multi-line phone system and utilize the hospital paging system.

  • Demonstrated customer service skills.

  • Ability to work in a busy environment.

Required

  • One year of clerical experience, or equivalency.

Qualifications (Preferred)

Preferred

  • Basic Life Support Health Care Provider card through American Heart Association or obtain within one month of hire may be required.

Working Conditions and Physical Demands

Employee must be able to meet the following requirements with or without an accommodation.

  • This position involves intermediate working conditions in a healthcare setting that may exert up to 50 pounds and may consistently require lifting, carrying, pushing, pulling, or otherwise moving patients or objects, such as medical equipment, while providing patient care. Workers in this position may be exposed to infectious diseases and may be required to function around prisoner or behavioral health patients.
