Job Summary

This position is responsible for delivering professional nursing care to patients by utilizing strong organizational and leadership skills to assess patient status; implements clinical nursing interventions to modify symptoms; and develops, implements and evaluates patient care plans to enhance and improve outcomes and ensures a safe and therapeutic environment.

Miller Grant Funding has been obtained to start Group Prenatal care at South Main Clinic for patients with gestational diabetes. Groups will meet every other week for the duration of the funding. No more than 1 year.

Responsibilities

Applies health behavioral knowledge to help develop group educational sessions for group prenatal care for women with diabetes

Participates in teaching/facilitating group prenatal care meetings/education sessions

Responsible for the day-to-day logistical maintenance of the group prenatal care project

Collects and maintains clinical database on participants (for reviewing blood glucose results, etc), and works to ensure patient confidentiality

Collects and helps to interpret blood glucose monitoring results in conjunction with the physician care team

Interacts with patients on the phone and in groups in both English and Spanish

Minimum Qualifications

Minimum qualifications:

Current licensure at Registered Nurse in the state of Utah

Current BLS

Preferences

Preference given to applicants with clinical experience in diabetes care, pregnancy/prenatal care, or group medical care experience (RN, MA, CNM ). The ideal applicant will have demonstrated effective outpatient clinical care management with regard to managing and tracking laboratory results, contacting patients, and communicating with the care team regarding individual plans of care.

Familiarity with RedCap and Epic. Familiarity with mobile applications for health.
