This position assumes responsibility for the management of patient medical records in support of patient care, quality and safety, in complex multimedia systems.
The incumbent has demonstrated skills in the functions of the job and has knowledge of regulatory/compliance standards.
This position has no responsibility for providing care to patients.
Scans and/or indexes medical records into the UUHC electronic medical record (EMR).
Performs quality control audits and correct errors on scanned documents.
Compiles, processes, and maintains medical records of hospital and clinic patients in a manner consistent with all UUHC requirements and standards.
Releases protected health information for direct patient care according to standards set by State, Federal and other regulatory agencies.
Provides exceptional customer service to patients and customers. Manage incoming phone calls to department and direct customers to appropriate resources.
Performs all morgue management duties, including verification of documentation, maintaining appropriate logs, notifying appropriate parties and receiving/releasing remains.
Initiates and completes electronic death certificates via state system and according to UUHC guidelines. Ensure accuracy and timeliness of information.
Determines appropriate document content and data elements necessary to be retained in the legal physical and electronic medical record.
Files, retrieves, or archives paper medical records according to UUHC Record Retention Schedule.
Be familiar with downtime procedures for all relevant HIM functions, such as scanning, electronic master patient index, and release of information.
Identifies and troubleshoots simple computer problems and escalates issues to appropriate resources as needed.
Assists in training and mentoring team members and HIM externs.
Performs other duties as assigned.
Knowledge / Skills / Abilities
Ability to perform basic computer skills to navigate in Electronic Medical Record systems.
Ability to provide appropriate customer service to patients and customers.
Ability to identify and troubleshoot simple computer problems and escalate issues to appropriate resource as needed.
Ability to communicate orally and in writing.
- Two or more years of clerical experience preferably in a health care environment or equivalent education.
Basic knowledge of Health Information.
Successfully trained in general HIM department functions.
Completion of Medical Terminology, Anatomy and Physiology within 6 months of hire.

- This is a position in an office setting. Shifts may involve 4 or more hours of walking and standing as well as sitting for extended periods of time. May be required to lift, carry, push, pull, or otherwise move objects of upto 50 lbs. Position is not exposed to adverse environmental conditions.
