Job Summary

The newly established Intensive Outpatient Clinic (IOC) is seeking a qualified Care Manager to be an integral part of an interdisciplinary team of medical professionals who collaborate and deliver exceptional patient-centered care for a targeted population identified as high risk for poor health outcomes and high utilization of healthcare services.

The Care Manager will be a key member of theIOCteam comprised of physicians, advanced practice clinicians, care managers, social workers and medical assistants. The Care Manager will work closely with team members to provide patient-centered care across the care continuum, interfacing with inpatient case managers while focusing on outpatient interventions.

Care management services include monitoring patient interventions to ensure progress toward desired outcomes, addressing patient and family needs, resolving obstacles to effective care, and coordinating care team members, patients and families.
This position may be required to access and administer medications within their scope of practice and according to State Law.

Responsibilities

• Coordinates care management process from patient’s entry into the Intensive Outpatient Clinic. Coordinates care and resources with physicians, social workers, and other team members to achieve optimal patient outcomes.


• Participates withIOCcare team and analytic team in identifying patients who are suitable for case management intervention based on health and demographic criteria established by theIOC.


• Monitors and documents quality of care to ensure patient care plan goals are appropriate and that they are understood and implemented. Routinely assesses client and family response to services, while also measuring care plan effectiveness and necessity. Also identifies patient needs, including those of an ethical and cultural nature, and ensures they are addressed.


• Facilitates cost effective outcomes by determining appropriate level of care based on diagnosis, severity, intensity of services required, and other relevant criteria, using national and regional length of stay standards and community norms.


• Assesses and discusses funding and insurance issues with client, family, and healthcare providers to enhance cost effective utilization of services and quality outcomes.


• Identifies actual and potential delays in service requests or treatment and communicates them to health care team so steps can be taken to eliminate or minimize delays.


• Works with inpatient case management team to plan appropriate and effective transition to outpatient care.


• Supervises technical support staff assigned to Intensive Outpatient Clinic.


• May set up patients’ follow up appointments and per care team design.


• May be required to complete home or site visits as required by the department.

• Ability to perform the essential functions of the job as outlined above.


• Demonstrated commitment and passion to serve high-risk patients with chronic, complex, and often intractable health concerns. Must consistently demonstrate professional demeanor and calm and courteous composure when interacting with patients and care team.

• Demonstrated team leadership, relationship building, critical analysis, and written and verbal communication skills.


• Knowledge of clinical standards and outcomes.


• Ability to provide care appropriate to the population served.

• Demonstrated independent judgment to assess and meet client needs. Ability to have meaningful outcome oriented dialogues with patients, families, various patient care disciplines, ancillary departments, health care and community agencies, and Health Science Center professional schools in coordinating care and services for patients.

• The staff member should be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned area.


• The individual should demonstrate knowledge of the principles of life span growth and development and the ability to assess data regarding the patient’s status and provide care as described in the department’s policies and procedures manual.

Minimum Qualifications

This position requires a Bachelor’s degree in Nursing or equivalency; current R.N. licensure in the State of Utah; two years of professional experience in a clinically related area; a Basic Life Support health care provider card; and demonstrated team leadership, relationship building, critical analysis, and written and verbal communication skills. Knowledge of funding resources and clinical standards and outcomes is also required. A Certified Case Management designation is preferred. Previous experience in clinical resource management activities and third party payer interactions is preferred.
