This position provides all the essential functions of a clinical pharmacist. In addition, the coordinator acts as a specialist in an assigned area of expertise, engages in education and researches projects, and provides clinical practice direction to other staff members. This position leads quality improvement and medication safety initiatives and plays a key role in the designing of department educational programs. This position will provide input for the evaluation of other staff members, but does not have direct supervisory responsibilities.
Solves medication related problems for patients in the medication use process (direct or indirectly). Demonstrates organizational problem solving skills. Facilitates team processes.
Identifies, engages, and coordinates department and organizational quality improvement activities to improve patient care and pharmacy operations. Uses and analyzes data effectively to improve clinical care.
Educates and trains residents, students, interns, colleagues and other health care professionals as well as supervises technicians, residents, interns or trainees in their job tasks. Identifies needs for staff development and develops a plan to address the needs.
Develops and implements medication use policy. Works effectively with interdisciplinary teams. Participates in hospital committees as assigned. Responsible for the medication use process and following medication use policy and all laws, regulations, and standards applicable to pharmacy practice.
Assesses and provides drug information and literature for varied audiences including patients, other health care professionals, and peers in addition to be able to document accurately for compliance, reimbursement or information purposes. Serves as a resource and role model for other staff.
Assists with department staffing, scheduling, planning and goal setting activities. Provides leadership for clinical services development. Leads problem solving teams.
Knowledge / Skills / Abilities
Ability to perform the essential functions of the job as outlined in the position description.
Ability and willingness to train department trainees (eg students, residents, new staff).
Ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
Demonstrated organizational and leadership capabilities.
Demonstrated knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
Demonstrated knowledge of the principles of life span growth and development and the ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.
An advanced degree (Pharm D. or M.S.).
Five years post-graduate clinical practice experience or equivalency.
Demonstrated human relation, effective communication, service orientation, organizational, and computer literacy skills.
Demonstrated leadership capabilities.
Additional qualifications may be required depending on area the pharmacist is primarily assigned to work. Examples of additional qualifications could include basic or advanced life-support training, residency training, or certifications.
Current licensure to dispense controlled substances in the State of Utah.
Current Pharmacist license in the State of Utah (Obtain within 90 days of starting residency)

Active membership in state or national pharmacy organizations.
